Permethrin is a topical medication formulated to treat scabies. a highly contagious skin infestation caused by the mite Sarcoptes scabiei. It spreads primarily through prolonged skin-to-skin contact and is associated with intense itching (especially at night) and a red, bumpy rash in areas such as the fingers, wrists, elbows, and groin.
Permethrin belongs to the synthetic pyrethroid class and works by disrupting the sodium channels in the nervous system of mites, leading to paralysis and death. It targets both adult mites and their eggs, helping eliminate the infestation and reduce inflammation caused by the immune response to the mites.
The cream is applied to clean, dry skin from the neck down, including between fingers/toes, under nails, and around skin folds (e.g., wrists, buttocks, genitals). The cream should be left on the skin for 8 to 14 hours, then thoroughly washed off. A second application may be needed after 7 to 14 days under medical supervision if symptoms persist.
Post-treatment itching is common and may persist for several weeks even after successful mite eradication. This is part of the body’s immune response and does not necessarily indicate treatment failure.
Permethrin is safe and well-tolerated in most patients. Mild side effects such as redness, stinging, or temporary itching may occur and usually resolve without intervention.
