General Information
Candesartan is an angiotensin II receptor blocker (ARB) that is used in tablet form to treat high blood pressure (hypertension) and to support heart failure management. By blocking the action of angiotensin II, a hormone that constricts blood vessels, candesartan helps relax and widen blood vessels, which reduces blood pressure and decreases the heart's workload.
It treats essential hypertension in adults and children over six, and, in combination with other drugs, improves outcomes in chronic heartfailure with reduced ejection fraction.
It blocks AT1 receptors so the hormone angiotensin II cannot tighten arteries or trigger saltwater retention. Vessels relax, the heart pumps against less resistance, and blood volume gradually falls.
Most adults start on 8 mg once daily; the dose is doubled every 4 weeks if needed up to 32 mg. In heartfailure the usual target is 32 mg once daily, titrated slowly.
The blood pressure-lowering effects of candesartan typically begin within 2 to 4 hours of taking the dose. However, it may take about 4 weeks of regular dosing to reach a steady state and for the full effects on blood pressure to be evident.
If you miss a dose of candesartan, take it as soon as you remember, but only if it's within the same day. If it's been more than 12 hours since the missed dose, skip it and take your next dose at the usual time. Do not take two doses to make up for the missed one.
Safety & Side Effects
The majority of users notice no problems. Some experience dizziness, headache, tiredness, or back pain during the first week. Laboratory checks may reveal a small rise in potassium or creatinine.
Rare but serious effects include kidney failure in people with bilateral renalartery stenosis, severe hypotension in volumedepleted patients, or angiooedema with facial swelling.

Caution is required in individuals with:
- Severe liver or kidney impairment (due to the risk of further deterioration in kidney function).
- Bilateral renal artery stenosis, as this condition increases the risk of acute renal failure when using ACE inhibitors or ARBs like candesartan.
In many cases, candesartan can be safely combined with other medications, but caution is required when used with the following:
- ACE inhibitors, aliskiren, or potassium-sparing diuretics, as combining them with candesartan can increase the risk of elevated potassium levels (hyperkalemia) and kidney damage.
- Nonsteroidal anti-inflammatory drugs (NSAIDs), which can reduce the effectiveness of candesartan in lowering blood pressure and increase the risk of kidney damage.
- High-dose trimethoprim, which may also raise potassium levels and cause kidney issues.
Noexposure after the first trimester can cause fetal kidney failure and skull defects. Switch to a safer alternative under guidance. Tiny amounts pass into breast milk; use is not recommended.
Moderate intake is fine but may accentuate dizziness; avoid standing abruptly after drinking.
