Evorel Sequi is a sequential combined hormone replacement therapy (HRT) that comes in the form of skin patches. It contains two hormones: estradiol, a form of estrogen, and norethisterone acetate, a type of progesterone (also called a progestogen).
Evorel Sequi is designed to relieve symptoms caused by the hormonal changes of menopause. It is usually prescribed if you are in perimenopause (your periods are irregular but not fully stopped) or if you are in the early stages after menopause and still have your womb.
Because Evorel Sequi follows a sequential treatment cycle, you use estrogen-only patches first, followed by combined estrogen and progesterone patches. This pattern often results in a monthly withdrawal bleed, similar to a light period.
Evorel Sequi is a transdermal HRT, meaning the hormones are delivered through your skin directly into your bloodstream. This provides steady hormone levels and avoids passing through your digestive system.
What is Evorel Sequi used for?
Evorel Sequi is used to treat menopause symptoms caused by low estrogen levels.
During menopause, your body produces less estrogen. This natural change can cause symptoms such as:
- Hot flushes
- Night sweats

How does Evorel Sequi work?
Evorel Sequi works by replacing the hormones that decline during menopause.
Estradiol (Estrogen)
Estradiol is chemically similar to the estrogen your body naturally produces. It helps to:

- Support mood and sleep
- Maintain bone strength

Norethisterone (Progestogen)
If you still have your womb, estrogen alone can cause the lining of your womb (called the endometrium) to thicken too much. Over time, this increases the risk of endometrial cancer.
Norethisterone protects the lining of your womb by preventing excessive thickening. In sequential HRT like Evorel Sequi, progesterone is added during part of the cycle, which may cause a predictable monthly bleed.
Because Evorel Sequi is delivered through the skin, it provides steady hormone absorption and may reduce certain risks compared to oral hormone tablets.
Who should not use Evorel Sequi?
You should not use Evorel Sequi if:
- You have or have had breast cancer
- You have estrogen-dependent cancer (such as cancer of the womb lining)
- You have unexplained vaginal bleeding
- You have untreated endometrial hyperplasia
- You have or have had blood clots (deep vein thrombosis or pulmonary embolism)
- You have a known blood clotting disorder
- You recently had a heart attack or stroke
- You have severe liver disease
- You are allergic to estradiol or norethisterone
Always inform your doctor about your full medical history before starting hormone replacement therapy.
Pregnancy and breastfeeding
Evorel Sequi is not a contraceptive.
You must not use Evorel Sequi if you are pregnant or think you may be pregnant. Pregnancy is still possible during perimenopause.
Evorel Sequi should not be used while breastfeeding.
If you suspect pregnancy while using the patch, remove it and contact your doctor immediately.
How to use Evorel Sequi and dosage
Always use Evorel Sequi exactly as prescribed.
Evorel Sequi contains two types of patches that are used in sequence:
- Estrogen-only patches (first part of the cycle)
- Combined estrogen and progesterone patches (second part of the cycle)
You typically change the patch twice weekly, following the schedule provided by your doctor.
How to apply:
- Apply to clean, dry, unbroken skin
- Place on your lower abdomen or buttocks
- Do not apply to your breasts
- Press firmly for at least 10 seconds
- Rotate application sites to prevent irritation
You may experience a predictable monthly bleed during the progesterone phase. This is expected with sequential HRT.

Safety & Side Effects
Evorel Sequi causes a predictable monthly bleed because the progestogen phase triggers shedding of the womb lining. This bleeding is expected with sequential HRT and usually resembles a light period.
Evorel Sequi, like other combined HRT treatments, slightly increases the risk of breast cancer when used for several years. Because it contains a progestogen, it protects the lining of the womb and reduces the risk of endometrial cancer compared to estrogen-only therapy. Ovarian cancer risk may be slightly increased.
Evorel Sequi may increase the risk of blood clots, particularly during the first year of treatment. Risk is higher in women with obesity, clotting disorders, prolonged immobility or a previous history of thrombosis.
Evorel Sequi is delivered through the skin, which avoids first-pass metabolism in the liver. Transdermal estrogen is generally associated with a lower effect on clotting factors compared to oral estrogen, although overall safety depends on individual risk factors.
Evorel Sequi may cause mild fluid retention in some women. Significant long-term weight gain is not consistently linked to transdermal HRT. Weight changes during menopause are influenced by hormonal changes and lifestyle factors.

Do not use Evorel Sequi if you:
- Have or have had breast cancer
- Have oestrogen-sensitive cancer such as cancer of the womb lining
- Have unexplained vaginal bleeding
- Have untreated endometrial hyperplasia
- Have or have had a blood clot in a vein (deep vein thrombosis or pulmonary embolism)
- Have a blood clotting disorder
- Have or recently had a heart attack, stroke or angina
- Have liver disease and your liver tests have not returned to normal
- Have porphyria
- Are allergic to estradiol, norethisterone acetate or any ingredient of the patch
Stop using Evorel Sequi and seek medical advice immediately if you develop:
- Signs of a blood clot (painful leg swelling, sudden chest pain, difficulty breathing)
- Yellowing of the skin or eyes (jaundice)
- Sudden migraine-type headache
- A significant rise in blood pressure
- Signs of severe allergic reaction
- Pregnancy
Hormone replacement therapy carries risks, including:
- Breast cancer
- Endometrial cancer
- Ovarian cancer
- Blood clots
- Stroke
- Heart disease
You should have regular medical reviews (at least once a year) to assess whether continuing treatment is appropriate.
Evorel Sequi is not suitable during pregnancy or breastfeeding.
Always use Evorel Sequi exactly as prescribed.
Each pack contains:
- Four Evorel 50 patches (estradiol only)
- Four Evorel Conti patches (estradiol and norethisterone)
Treatment cycle:
Weeks 1 and 2: Use the four estradiol-only patches
Weeks 3 and 4: Use the four combined patches
Apply a new patch twice weekly. Change on the same two days every week. One patch is worn for three days and the next for four days.
Apply the patch:
- To clean, dry, hairless skin below the waist
- Do not apply to or near the breasts
- Do not apply to irritated or damaged skin
- Do not use creams or moisturisers before application
- Rotate application sites
If a patch falls off, replace it and continue your normal change schedule.
If you forget to change a patch, apply a new one as soon as you remember.
If you need surgery, you may need to stop HRT 4 to 6 weeks beforehand to reduce clot risk.
Like all medicines, Evorel Sequi can cause side effects.
Diseases reported more often in women using HRT include:
- Breast cancer
- Endometrial cancer
- Ovarian cancer
- Blood clots
- Stroke
- Heart disease
- Probable memory loss in women starting HRT after age 65
Very common:
- Skin irritation, redness or itching at the application site
Common:
- Headache
- Nausea
- Stomach discomfort
- Breast pain
- Vaginal bleeding or spotting
- Weight gain
- Fluid retention
- Mood changes
Uncommon:
- Dizziness
- Palpitations
- Breast lumps (non-cancerous)
- Thickening of the womb lining
Rare or serious side effects include:
- Blood clots
- Stroke
- Severe allergic reactions
- Liver problems
- Stevens–Johnson syndrome
Remove the patch and seek urgent medical help if you experience symptoms of blood clot, stroke, severe allergic reaction or liver disease.
- Keep out of the sight and reach of children
- Store at room temperature (do not store above 25°C)
- Do not use after the expiry date
- Do not use if the protective pouch is open
- Do not dispose of medicines via wastewater or household waste. Ask your pharmacist how to dispose of them properly
